7 Sit in Sitz bath twice a day, hot tub bath with savlon or betadine in it

8 Take milk with haldi and honey twice a day

11 Do not strain in defecation

12 Change underwear daily

13 take cloves water daily

भगन्दर होने पर मरीज को गुदा में दर्द महसूस होता है । ये दर्द पस एकत्रित होने के कारण होता है । फोड़ा फटने पर पस निकल जाता है तथा दर्द कम या खत्म हो जाता है । इसके अतिरिक्त मरीज खुजली आँव खून आना बुखार सूजन की शिकायत भी करते हैं ।

बीमारी का पता एक गुदा रोग विशेषज्ञ देख कर तथा अपनी अंगुली से चेक अप करके कर सकता है (DRE) MRI द्वारा इसका पूरा फैलाव देखा जा सकता है ।

Syphilis test may be VDRL test full form  (venereal disease research laboratory test), RPR (Rapid plasma reagin test) and more confirmatory tests are TPHA(T pallidum hemagglutination assay) and FTA-ABS (fluorescent treponemal antibody absorbed).

Syphilis treatment by Benzathine penicillin G 2.4 million units administered intramuscularly one injection to 3 shots. We may also give Tetracycline, Azithromycin and Doxycycline.

HIV infection is related with sex . Its transmission is also possible with blood and from mother to child. Today I will discuss about probability of acquiring HIV infection with various sexual and other activities.

Risk of getting HIV per 10000 exposures in blood transfusion is 9250, by needle sharing is 63 and by accidental needle prick is 23.

Risk of getting HIV per 10000 exposures without protection in receptive anal intercourse is 138, in insertive anal intercourse is 11, in receptive penile vaginal intercourse is 8, in insertive penile vaginal intercourse is 4. Risk is low in receptive and insertive oral intercourse.

Risk is negligible in Biting, Spitting, Throwing body fluids semen saliva, or sharing sex toys.

Factors that may increase risk of HIV transmission

STD, acute and late stage of HIV infection, high viral load, if lady is mensurating.

In vaginal sex more chances of infection if sex is tried in - Doggy style, Wheelbarrow style, Cowgirl woman on top style, Froggy style.

The risk of HIV infection in child from mother is 30-45% during pregnancy, labour and breast feeding.

If blood touches the undamaged unbroken skin there is no HIV risk.

मेरी क्लीनिक में आने वाले मरीजों की ऑपरेशन से पहले जब मैं जांच करवाता हूँ तो उसमें से कुछ मरीज एचआईवी पॉजिटिव निकलते हैं। मेरे साथ साथ वो भी अचम्भा करते हैं कि उन्हें ये बीमारी कैसे हो सकती है, उन्हें तो कोई तकलीफ नहीं है। उन्होंने तो कभी किसी अजनबी के साथ सेक्स नहीं किया है। 

क्या आपको मालूम है कि सेक्स के अतिरिक्त भी बहुत सारे तरीके हैं जिनसे एचआईवी फैल सकती है। 

आज में आपसे उन सभी तरीकों की बात करूँगा जिनका ध्यान रखकर आप एचआईवी एड्स होने से अपनेआपको बचा सकते हैं। 

1. एचआईवी होने का सबसे प्रमुख तरीका है असुरछित सेक्स। जहाँ तक संभव हो एक ही साथी के साथ सेक्स करें। ये भी ध्यान रखें कि आप के साथी का भी एक ही सेक्स पार्टनर हो। अगर आप किसी नए साथी से सेक्स कर रहें हों तो उसकी एचआईवी निगेटिव रिपोर्ट अवश्य देख लें। याद रखें हर प्रकार के सेक्स से एचआईवी फैल सकता है ।

2 . हर बार सेक्स करते समय कंडोम अवश्य पहनें। कंडोम सही प्रकार से पहनें। पहनने से पहले उसकी एक्सपायरी डेट देख लें। यह भी देखें कि वो फटा हुआ तो नहीं है। सेक्स के बाद उसे सही प्रकार से उतारें। अगर एक समय में एक बार से अधिक सेक्स कर रहे हैं तो भी हर बार नया कंडोम अवश्य पहनें। दुबारा एक ही कंडोम को न पहनें। एक बार में दो कंडोम न पहनें ।

3. कंडोम लेटेक्स Latex या Polyurethane से बना होना चाहिए। अन्य किसी भी प्रकार का कंडोम न पहनें। 

4. कंडोम को बाहर से चिकना करने के लिए पानी या सिलिकॉन बेस से बने लुब्रीकेंट का प्रयोग करें। लुब्रीकेंट को कंडोम के बाहर लगाएं, तथा साथी के जननांगों पर लगाएं। लिंग के ऊपर लुब्रीकेंट न लगाएं। 

5. याद रखें थूक या saliva एक अच्छा लुब्रीकेंट नहीं है। 

6. तेल आयल बेस वाले लुब्रीकेंट न प्रयोग में लाएं।  ये लेटेक्स को नुकसान पहुचांते हैं, जैसे- 

खाना पकाने के तेल, बेबी आयल, कोकोनट आयल, मिनरल आयल, 

पेट्रोलियम जेली, वेसिलीन, लोशन, कोल्ड क्रीम, मक्खन, आदि 

7. इंजेक्शन के लिए एक बार इस्तेमाल होने वाली सुइयों का प्रयोग करें। उसे मोड़ कर तुरंत फेंक दें। किसी दूसरे की सुई का प्रयोग न करें। छोटे चिकित्सक एक से सुई से सबको इंजेक्शन लगाते रहते हैं। 

8. अन्य यौन रोगों का परीछण करवा लें तथा उसका समुचित इलाज ले लें। एसटीडी होने पर HIV होने की सम्भावना बढ़ जाती है। 

9. सेक्स से पहले अत्यधिक नशे का या ड्रग्स का सेवन न करें। नशा करने के कारण आप कंडोम का सही प्रयोग नहीं कर पाते हैं। 

10. अगर आप शरीर पर टैटू tatto बनवा रहे हैं तो नयी नीडल से ही बनवाएं।

11.  अगर आप खून चढ़वा रहें हों तो ब्लड बैंक से चेक करके ही लें। 

12. नाई की दुकान में हर बार नया ब्लेड प्रयोग में लाएं। 

13. गर्भावस्था में महिला का hiv टेस्ट अवश्य करवा लें। पॉजिटिव आने पर मां तथा बच्चा दोनों का इलाज जरुरी है। 

14. अपना HIV Test करवा लें, पॉजिटिव आने पर इलाज प्रारम्भ कर दें। दवा से बीमारी दबी रहती है इसलिए इसे कभी भी बंद न करें। सेक्स में कंडोम का प्रयोग जरूर करें।
