नवीन वैज्ञानिक शोधों के अनुसार बवासीर वास्तव में कुशंस हैं, जो गुदा मार्ग को कसकर बंद रखते हैं तथा हवा, पाद, पानी, मल आदि को बिना दिमाग के संकेत के बाहर नहीं आने देते हैं।  जब यह कुशंस ढीले पड़ जाते हैं, लटकने लगते हैं, तो इनके अंदर रक्त वाहिकायें  बन जाती हैं।  रगड़ लगने पर इन ऊतकों से खून रिसता है या अत्यधिक सूजन आ जाती है।  पुरानी धारणाओं के विपरीत यह खून धार से धमनियों (artery ) से बहुत तीव्र गति से निकलता है।  सफेद कमोड पर सुर्ख लाल रंग देखकर मरीज को चक्कर भी आने लगते हैं।

लेजर की मशीन अभी भारतवर्ष में नहीं बन रही है।  मैंने भी अपनी मशीन इजराइल से मंगाई है। 

2 कोई चीरफाड नहीं

'लेजर सर्जरी' शब्द के बावजूद इसमें कोई भी चीर फाड़ नहीं की जाती है।  किसी प्रकार का चीरा नहीं लगाया जाता है।  सिर्फ एक पतला सा लेजर का धागा हमें बवासीर के मस्सों से छुटकारा दिला देता है। 

3 अत्यधिक रक्तस्राव नहीं

क्योंकि किसी प्रकार का चीरा हम नहीं लगा रहे हैं इस कारण किसी भी प्रकार का खून नहीं बहता है।  आम बवासीर की सर्जरी में हमें गुदा मार्ग को बहुत जगह से काटना पड़ता है, इस कारण रक्त स्राव बहुत होता है।  यह रक्त स्रावऑपरेशन के पश्चात भी बहुत दिनों तक होता ही रहता है। 

4 गुदा मार्ग संकुचन नहीं होना ( Less Surgical Complications )

क्योंकि ऑपरेशन के दौरान गुदा मार्ग  को हमें काटना पड़ता है, घाव भरते समय यह सिकुड़ता है, इस कारण कई बार गुदा मार्ग संकुचित हो जाता है Anal Stenosis   ज्यादा संकुचित होने पर उसका भी ऑपरेशन करना पड़ सकता है। 

5 दर्द

क्योंकि हम किसी भी प्रकार की चीर फाड़ नहीं कर रहे हैं इसलिए गुदा मार्ग  में किसी भी प्रकार का दर्द नहीं होता है।  सामान्य ऑपरेशन के बाद कुछ हफ्तों तक मरीज को दर्द बना रहता है तथा उसे दर्द की दवाइयां नियमित रूप से खानी पड़ती है। 

6 लेजर में कम समय लगना

सामान्य ऑपरेशन में लगभग 1 घंटा समय लगता है।  लेजर द्वारा सभी कार्य  आधे घंटे से भी कम समय में किया जा सकता है।  कम समय लगने के कारण मरीज को जल्दी से स्वास्थ्य लाभ होता है।

7 घाव को जल्दी से भरना

सामान्य ऑपरेशन में बवासीर का घाव भरने में हफ्तों लगते हैं।  लेजर में हम चूंकि कोई चीर फाड़ नहीं करते हैं मरीज जल्दी से स्वस्थ हो जाता है। 

8 अन्य उतको पर कोई दुष्प्रभाव नहीं

लेजर का प्रभाव बहुत गहरा नहीं होता है।  यह केवल उसी हिस्से में कार्य करती है जहां इसका उपयोग किया जाता है।  इसके विपरीत सामान्य ऑपरेशन में कई बार अन्य अंगों  को भी नुकसान पहुंच सकता है।  यदि गुदा के चारों तरफ लिपटी मांसपेशियां कट जाती  हैं तो शरीर की  पाद  एवं मल रोकने की क्षमता घट जाती है। 

9 सुनिश्चितता

लेजर सुनिश्चितता  से केवल बवासीर को ठीक करता है, आसपास के ऊतकों  को कोई नुकसान नहीं पहुंचाता है।  यह इसका बहुत ही बड़ा पॉजिटिव प्वाइंट है। 

10 इंफेक्शन का ना होना

चीर फाड़ के ना होने के कारण लेजर में किसी प्रकार के इंफेक्शन, सेप्टिक या मवाद  पड़ने की उम्मीद नहीं होती है।  

सामान्य ऑपरेशन में इन्फेक्शन होना एक आम बात है इस कारण मरीज को बहुत दिनों तक एंटीबायोटिक्स खाने पड़ते हैं।

 एनल फिशर क्या होता है, इसका बचाव तथा उपचार 

गुदा मार्ग  में कोई भी परेशानी होने पर पीड़ित रोगी चिकित्सक से बवासीर का इलाज करने का आग्रह करता है।  वास्तव में बवासीर के अतिरिक्त भी अन्य कई बीमारियां हैं जो गुदा मार्ग पर हो सकती हैं - जैसे एनल फिशर, भगंदर, वार्ट, कैंसर, खुजली, खाज, नासूर आदि

एक गुदा रोग विशेषज्ञ अथवा प्रॉक्टोलॉजिस्ट ही बीमारी को सही प्रकार से पहचान सकता है।  यह इसलिए भी महत्वपूर्ण है कि बवासीर तथा अन्य रोगों का उपचार एवं चिकित्सा अलग-अलग है। 

एनल फिशर  के रोगी को शौच जाते वक्त गुदा में दर्द का अनुभव होता है।  कुछ रोगी  तीव्र जलन की भी शिकायत करते हैं।  प्रारंभ में यह दर्द शौच जाने के समय ही होता है।  धीरे-धीरे दर्द की तीव्रता बढ़ती जाती है।  कुछ समय पश्चात उपचार न लेने पर, दर्द शौच जाने के बाद भी होने लगता है।  मरीज घंटों तक इस दर्द को महसूस करता है।  बाद में शौच जाते समय प्रारंभ हुआ दर्द दिनभर ही बना रहता है।  दर्द के कारण रोगी को चक्कर आते हैं, उसकी टांगे कांपने लगती हैं।  रोगी शौच करने जाने से भी डरने  लगता है।  शौच  में बैठते ही वह उठ  खड़ा होता है, इससे बड़ी आत में मल  एकत्रित होकर  सूखने लगता है। 

कुछ रोगियों को मल के साथ तथा बाद में खून भी आता है।  यह खून प्रारंभ में मल के ऊपर एक लाइन की तरह लग कर आता है।  रोगी पूछने पर बताता भी है कि उसने मल के ऊपर रक्त  की लाइन बनी देखी है।  उपचार न लेने पर एनल फिशर का घाव गहरा होता जाता है।  इसमें से शौच के समय बहुत रक्त आता है। 

कुछ फिसर के रोगियों को गुदा मार्ग पर खुजली होती है।  उन्हें जोर से  खुजाने का मन भी करता है।  कुछ रोगियों को गुदा मार्ग पर भारीपन सा लगता रहता है। 

फिशर का उपचार न लेने पर किसके ऊपर बाहर की तरफ सूजन आने लगती है, जिसे हम 'सेंटिनल पाइल'  के रूप में जानते हैं।  यह वास्तव में बवासीर नहीं है, सूजन है।  रोगी इससे अनभिज्ञ इस  सूजन को बवासीर का मस्सा ही समझते हैं।  एक योग्य गुदा रोग विशेषज्ञ इसकी पहचान सही प्रकार से कर सकता है। 

घाव  के कारण गुदा मार्ग तनाव में बना रहता है।  रोगी अपनी गुदा को कसकर बंद रखता है।  इसी कारण गुदा का अंदरूनी परीक्षण करना भी संभव नहीं हो पाता है। 

हमारा गुदा मार दो तरह की गोल मासपेशियों से घिरा रहता है।  इसमें से अंदर वाली मांसपेशी हमेशा तनाव में रहती है तथा गुदा मार्ग को दबा कर रखती है, ताकि उसमें से वायु, पाद, मल  बाहर न निकल  सके।  

बाहरी मांसपेशी हमारे दिमाग के अनुरूप ढीली या कड़ी हो सकती है। 

फिशर होने का मूल कारण

फिशर होने का मूल कारण है - कब्ज।  

मल हमारी बड़ी आंतों में पड़े पड़े सूख जाता है तथा उसका आकार बड़ा हो जाता है।  इस प्रकार के मल  को जब हमारी आंतें  जोर लगा कर बाहर निकालती हैं, उस समय गुदा मार्ग छिल जाता है या फट जाता है।  गुदा मार्ग की झिल्ली केवल छिल भी  सकती है तथा वह पूरी तरह फट भी सकती है। 

अगर झिल्ली केवल ऊपर से ही छिली  होती है तो कब्ज को सही रखकर तथा अन्य दवाइयों से यह घाव भर सकता है।  अन्यथा हमको उसका ऑपरेशन ही करना पड़ता है। 

घाव के कारण गुदा मार्ग  को घेरी अंदरूनी मांसपेशियां  और अधिक तनाव में आ जाती हैं।  इस कारण रक्त का प्रवाह भी इस हिस्से में कम हो जाता है।  इससे भी फिशर का घाव भरने में परेशानी होती है।  ज्यादातर यह घाव पीछे की तरफ होते हैं।  बहुत कम लोगों में यह ऊपर की तरफ होते हैं। 

दवाइयों द्वारा उपचार

लगभग 80% एनल फिशर जो गहरे नहीं होते हैं दवाइयों द्वारा ठीक हो जाते हैं।  इसका उपचार कब्ज को सही करके ही संभव है।  कब्ज से फ़िशर  होता है, फिशर से और कब्ज होता है, कब्ज पुनः  फिशर के घाव को हरा कर देता है।  इस चक्र को तोड़कर तथा कब्ज का उपचार कर हम रोगी को रोग मुक्त कर सकते हैं। 

भोजन 

कब्ज से मुक्ति पाने के लिए हमें अधिक रेशे युक्त भोजन, फल तथा सलाद का सेवन करना चाहिए।

सिट्ज़ बाथ 

फिशर में गुनगुने पानी से भरे टब में बैठकर सिकाई करने से भी बहुत आराम मिलता है।  सिकाई अंदरूनी मांसपेशी के तनाव को भी  कम करती है।  सिकाई करते समय हमारा गुदा मार्ग तथा कूल्हे  गुनगुने पानी में डूबे रहने चाहिए। 

इंजेक्शन द्वारा उपचार  

कुछ चिकित्सक बोटूलिनम टॉक्सिन के इंजेक्शन भी गुदा मार्ग  की मांसपेशियों में लगाते हैं।  लेकिन एक बार असर आने  पर, लगभग 3 महीने बाद इसका असर धीरे धीरे खत्म हो जाता है। 

मलहम 

कुछ मलहम भी  फिशर के लिए बाजार में उपलब्ध है।  लेकिन उनका प्रयोग चिकित्सीय सलाह के बिना कदापि भी नहीं करना चाहिए। 

ऑपरेशन 

लेजर द्वारा फिशर का ऑपरेशन सहजता से किया जा सकता है।  इससे दर्द भी बहुत कम होता है, शाम तक रोगी की छुट्टी भी हो जाती है।
